A personal medical book is required for representatives of professions involved in the production, storage, transportation and sale of food and water, education and upbringing of children, public utilities and consumer services.
The list of activities is established by order of Rospotrebnadzor No. 402. According to the legislative act, a personal medical book should be for employees who are involved in the following areas:
- circulation of food products and food raw materials in the food industry, trade, catering establishments;
- educational he alth activities for children and adolescents in kindergartens, schools, colleges;
- utilities such as hairdressers, hotels;
- sale of manufactured goods (clothing, toys, household appliances, car parts);
- drivers of urban passenger transport (bus and trolleybus depots, metro, shuttle buses);
- medical workers.
Personal medical book is kept by the administration of the organizationor an individual entrepreneur, issued to an employee:
- on demand;
- when leaving work.
Renewal of the medical book takes place according to the schedule established for each speci alty. The document must be transferred to the next place of work.
Registration and clearance
Since 2001, the Moscow Center for Hygiene and Epidemiology has organized an electronic register of issued personal medical books, which records the results of a professional hygiene check.
According to the document, a personal medical book was equated with documentation of strict accountability. After passing the mandatory preventive medical examination, the employee is obliged to enter the results in the medical book. The document reflects hygienic certification.
Who issues medical books?
Only organizations authorized by Rospotrebnadzor - centers of hygiene and epidemiology - can issue a document. The official status of the personal medical record is confirmed:
- holographic;
- seal of the Rospotrebnadzor organization issuing the document;
- approval of the certification result with the stamp of an organization authorized by Rospotrebnadzor in order No. 402.
The real medical book in Moscow is not available for free purchase and sale. When undergoing hygienic training, the document is handed out and must be registered.
Procedure for registration
To issue a medical book, the employee must present:
- a written application for extradition;
- passport;
- photo (3 by 4 cm);
- referral from the employer with the name of the place of work;
- registration certificate in Moscow or the Moscow region (this applies only to non-residents).
Documents can be submitted personally by the applicant or an authorized representative with a power of attorney.
The completed form of a personal medical book obliges the applicant:
- Pass examinations at licensed medical institutions.
- Study a vocational hygiene training program.
Persons who have undergone hygienic training and received a doctor's opinion on the result of a medical examination are allowed to be certified. The results are recorded in a personal medical book in accordance with the procedure approved by the order. Terms of registration
To issue a medical book quickly means spending at least 5-7 days. One-day and even half-hour filling is tantamount to forging a document. The rules of procedure are established by law. Any acceleration of the process will be equated with a violation of the law.
Obtaining and renewing medical records involves several stages in a specialized clinic or center:
- Fill out the bypass sheet, which is issued at the registration desk.
- Pay for a medical examination.
- Take tests, do x-rays, bypass doctors.
- Contactregister with a completed bypass sheet, receive a tear-off coupon for passing a medical examination. It indicates the time of receipt of the medical book.
- Come to the hygienic certification and pick up the document.

Specialized centers
Medical centers accredited by Rospotrebnadzor and the Moscow Center for Epidemiology provide a full range of services in accordance with the law. By order No. 302n, these medical institutions conduct medical examinations and draw up books with entry in the state register.
Officially, a medical book in Moscow is not issued within one day just because getting the results of a blood test takes more time. List of mandatory examinations
The list of doctors to bypass and tests depends on the specialization of the worker:
- The standard set of examinations for most speci alties includes: x-ray, blood test for syphilis, gonorrhea and Trichomonas. Need a bypass with a therapist and a dermatovenereologist.
- Occupations that come into contact with children, patients and food will need dental and otolaryngological examinations. Mandatory throat swab for Staphylococcus aureus, examination for helminths, enterobiasis and typhoid fever.
- Hazardous worker will be required to take a biochemical blood test to determine the level of bilirubin, enzymes and liver proteins.

- lecture or hygiene education expenses;
- medical book form;
- tutorial.
The data from the official website of Rospotrebnadzor determine how much a medical book costs when it is initially issued. This complex cost an average of 800 rubles at the beginning of 2017. A referral to a dermatovenereologist, blood tests are given to polyclinics at the place of residence. - blood, urine, infections and throat swabs - up to 10 days;
- HIV and hepatitis test - 90 days;
- bacteriological cultures and cytology - up to 20 days;
- analysis for helminths - 30 days.
Examinations by doctors are valid for a month, as are the results of ultrasound of the abdominal organs. Mammograms should be done every two years.
