Pharmacology
Polyvalent protease inhibitor, exhibits antiproteolytic, antifibrinolytic and hemostatic effects.
It has a wide range of inhibitory effects and for this reason can be used not only as an antifibrinolytic agent, but also as a preventive and therapeutic drug for pathologies of other enzyme systems. Activity is expressed in inactivating units (IU) as well as in antitrypsin units (ATrU).
The presence of antiprotease activity establishes the high efficiency of aprotinin in pancreatic lesionsglands and other conditions accompanied by a significant entry of kallikrein and other proteases in plasma and tissues. It reduces the fibrinolytic dynamism of the blood, delays fibrinolysis and has a hemostatic effect in coagulopathy. The blockade of the kallikrein-kinin system makes it possible to use it to prevent and cure various forms of shock.
What is Kontrykal used for?
Instructions for the use of "Kontrykal" (there are no tablets, there are only ampoules) the main indications are as follows:
- acute inflammation;
- prevention of postoperative pancreatitis and fat embolism;
- bleeding and hemorrhage due to hyperfibrinolysis (posttraumatic, postoperative);
- severe hemorrhagic complications during thrombolytic therapy;
- shock states.
Contraindications
According to the instructions, the drug cannot be used in case of hypersensitivity to the initiating substance, while waiting for the baby (namely, before the beginning of the fourth month). Care should be taken when using "Kontrykal" by patients with polyvalent allergies. Already in the second and third trimester, the medication can be used only for special indications. The agent is not compatible with antibiotics and solutions containing lipids, amino acids, polysaccharides. You can only buy with a prescription. Cautions

Method of use and dosage

It is administered intravenously, in a stream, slowly, up to 20-30 thousand units (2-3 ampoules) per day. In the future, a similar dose is administered intravenously drip. Such treatment should be continued until the patient's condition improves.
Instructions for the use of "Kontrykal" in pancreatitis indicates that in order to prevent exacerbations of the disease withsurgical actions, an initial dose of 20 thousand units is administered, then 10 thousand units up to 4 times a day, but every 6 hours.
In a state of shock, a massive portion of 200 thousand units will be simultaneously introduced (intravenously, by stream, slowly), then - 140 thousand every 4 minutes.
For the prevention of fat embolism, 200 thousand are administered every day, in the 1st period - by stream, in the following days - by drip.
Instructions for the use of "Kontrykal" (droppers) are as follows: in case of bleeding during childbirth, it is administered from 700 thousand up to 1 million units. Then they introduce again 700 thousand each (up to the stage of stopping bleeding).
The daily portion for a child is calculated based on the body weight of a young patient - 14 thousand per 1 kg of weight.
Dicinone
"Dicinone" belongs to the drugs that affect the cardiovascular system. Antihemorrhagic agent. It inhibits the decomposition of mucopolysaccharides of the vascular wall (anti-hyaluronidase dynamism), due to which it restores its permeability during painful processes. It also acts hemostatically by increasing the rate of creation of the primary thrombus (the effect is determined by the stimulation of the formation of tissue thromboplastin). It does not affect prothrombin time and does not have hypercoagulable properties. The onset of action is 5-15 minutes after injection and 1-2 minutes after oral administration. The duration of action is 4-6 hours. After course treatmentsubstance, the result is stored for 5-8 days.
Tachocomb
"Tachocomb" is considered an absorbent hemostatic drug for topical use. "Tachocomb" consists of a collagen plate covered with elements that help blood to clot. Upon contact with a bloody wound or other body fluids, the coagulation factors in the covering membrane are emptied, and thrombin transforms fibrinogen into fibrin.
